Greene County Launches COVID Vaccine Notification Request Form on GreeneGovernment.com

Greene County has created a new COVID Vaccine Notification system to help expedite and organize requests to receive vaccinations under the NYS guidelines.

The Vaccine Notification Request form was created specifically for Greene County residents and those who work here that qualify for the phased roll-out of vaccines.  At this time, all county residents who are eligible to receive vaccinations under Phase 1b (including Seniors Aged 65+) are strongly urged to add their information to the database so they may be contacted quickly as vaccines become available.

Currently, Seniors can only receive vaccines through authorized Pharmacies and Health Care Providers.  Should vaccines become available to the county to inoculate Seniors directly, this list will enable us to communicate quickly to all who have signed up. Greene County will continue to advocate for better access for Seniors through communications with NYS and local vaccine providers.  As additional information becomes available, Seniors on this list will receive notification by eMail or phone.

Phase 1b currently includes:

  • Firefighters & Fire Investigators
  • State and Local Law Enforcement
  • Corrections Officers and Personnel
  • Teachers, Instructors and School Workers
  • Childcare Providers
  • Public Transit Workers
  • Individuals Living or Working in Homeless Shelters
  • Restaurant Workers
  • Taxi Drivers
  • Seniors Aged 65+

“This new database will serve multiple purposes,” says Legislature Chairman Patrick S. Linger. “Even though it will be initially used to facilitate COVID vaccination notifications, it is also being used as a resource for future communications to quickly reach our residents – especially seniors – by both eMail and phone.”

“If eventually, the State does authorize Greene County to administer vaccine to Seniors”, Linger continues, “This database will be in place for future communication to all who have submitted the notification request form on our County website.”

Everyone who lives or works in Greene County is strongly urged to add their information when they become eligible under the NYS phased distribution plan. At this time, only persons who qualify under Phase 1b (including Seniors Aged 65+) should fill out and submit the on-line COVID Vaccination Notification Request Form

Greene County will continue to inform our residents when more groups become eligible under the NYS plan.

Residents are encouraged to utilize this Notification Request Form as well as any additional registration options available from the State and participating pharmacies.
