It means that the cost of your services is based on your income. If you have no insurance and do not qualify for the Family Planning Benefit Program you may go on the sliding fee scale. You do need to provide proof of income. If you do not have proof of income or choose not to provide proof of income your services must be charged at the top income of the sliding fee scale.
